Resident FAQ’s for Submitting ACOS Annual Reports

  1. Which Operative Log Report should I submit?
    1. Operative reports are submitted though the Resident Evaluation of the Program Form. Residents should download a PDF file and submit the correct Operative Report based on the system used for their OGME training level and the 2016-2017 Training Year.
      •   If using the ACOS Op-Log system, please submit the "Resident Operative Report"
      •   If using the ACGME Op-Log System, please submit the "Experience by Role" report
      •   If you are a graduating resident using the ACGME op-log system, please submit the "Experience by Year" report 
         
  2. Can I submit a paper Annual Report form?
    1. No, all documents are submitted electronically via the iCohere member community.
  3. How do I know if you received my Resident Evaluation of the Program Form?
    1. If your form has been completed and successfully submitted, you will see the completed form in your iCohere member community account under “Annual Report Forms”/ “Resident Evaluation of the program form”/”Search Results”/”Mine”/”Go”. If you can see the completed form, so can we!
  4. I have a new Program Director. How will they complete the Program Directors Checklist of the Resident form?
    1. We have notified each program director of their requirements for the Annual Resident Report. If your Program Director needs assistance logging in to the member community, iCohere please share Jessika Brown’s contact information and she will assist them, [email protected].
  5. I believe I am a member of the ACOS, why am I being charged a $300.00 non-member review fee?
    1. Questions regarding your membership status should be directed to Sonjya Johnson, [email protected].
  6. I am having a difficult time accessing my account.
    1. Please contact the ACOS 1.800.888.1312 x108. The operator will help you access your account.
  7. I logged onto iCohere and there is not an option for Annual Report Forms, who do I contact?
    1. Annual Report forms are not available using tablets or cellphones, for additional assistance please contact Jessika Brown, [email protected] or 1.800.888.1312 x 103
  8. When will I know the status of my training year?
    1. A result email notification will be sent Mid to Late September, after the RESC meets to review your reports. At that time, you will be able to logon to your www.facos.org account and print or save your Training Status Letter.
  9. My program has applied for accreditation with the ACGME. Am I still required to submit Annual Resident Reports to the ACOS?
    1. Programs who have not applied for ACGME accreditation, or received ACGME pre-accreditation, continued pre-accreditation or initial accreditation status and have NOT closed with the AOA are REQUIRED to submit annual resident reports. Residents and programs do not submit Annual Resident Reports if the program has received ACGME Initial Accreditation AND has officially closed with the AOA.

Program Directors FAQ’s for submitting ACOS Annual Reports

  1. My program has applied for accreditation with the ACGME. Am I still required to submit Annual Resident Reports to the ACOS?
    1. Programs who have not applied for ACGME accreditation, or received ACGME pre-accreditation, continued pre-accreditation or initial accreditation status and have NOT closed with the AOA are REQUIRED to submit annual resident reports. Residents and programs do not submit Annual Resident Reports if the program has received ACGME Initial Accreditation AND has officially closed with the AOA.

    1. If your form has been completed and successfully submitted, you will see the completed form in your iCohere member community account under “Annual Report Forms”/ “Resident Evaluation of the program form”/”Search Results”/”Mine”/”Go”. If you can see the completed form, so can we!

4.     I am a new Program Director/coordinator. How will I complete the Program Director’s forms?
